Take for example how a dental shot (Novocaine injection) is delivered at CSD compared to a typical dental office. First CSD dentists always start a Novocaine injection with the narrowest needle possible; if a wider, longer needle may be necessary it will not be used until after the patient is already numb in that area. The actual injection of the Novocaine into the patient is delivered very slowly because many people experience pain from a sudden large volume of injected Novocaine.
CSD offers many different types of Novocaine because some people may not get numb with the more common brands but will respond quite well to the lesser known but more expensive brands.
The amount of numbness achieved for dentistry can vary tremendously because of the different chemistry of the different brands. Novocaine is a commonly known brand and so many people incorrectly think that all dental anesthetic is the same.
Some areas of the mouth, like directly under the nose or the palate, are much more sensitive to dental injections.
In these cases CSD dentists will start the application of Novocaine in a less sensitive area first and give it time to get numb. Then a series of smaller injections will lead to the sensitive area but always remaining within the leading edge of the anesthetized gums. This results in a patient feeling minimal discomfort during injection in very sensitive areas of the mouth. This technique requires extra time and the desire & skill to offer pain free dentistry.
Sometimes it is appropriate to wait five or ten minutes between injections; sometimes a patient may need 15 20 minutes for their teeth and gums to become fully numb.
All of this takes time and cannot be accomplished in an insurance dental office that has to put a new patient in the same chair every thirty minutes. Tooth Pain and Root Canal Therapy
Root Canal Therapy Does Not Have to Hurt.
His procedure has a bad reputation for two reasons: 1) people frequently wait too long after tooth pain begins before they seek treatment and 2) many dentists don't perform root canal therapy particularly well. Conversely, if root canal therapy is initiated before, or at the onset of, pain then it can be expected to be relatively pain free.
The Center for Special Dentistry (CSD) in New York City has several Endodontists (Root Canal Specialists) on staff to treat our patients.
Root Canal Therapy is usually completed in one office visit in under an hour. Patients should ask to see the final x-ray following root canal therapy to learn to differentiate good root canal therapy from one that will likely need retreatment later. It's better to get something done right the first time.
Root canal therapy is NOT painful when properly performed in a tooth that has not yet reached the point of becoming a painful emergency AND when it is performed well by a CSD Endodontist.
Some people mistakenly wait until they have severe pain before seeking root canal therapy. They are the cause of the pain, not the procedure itself. An important lesson is therefore to contact our NY Endodontists now for root canal therapy if you think a tooth is becoming more painful; don't wait for the emergency.
Braces and Adult Orthodontics

We focus on adults because mature patients logically have a different set of orthodontic wants and needs than kids. Orthodontic teeth movement is coordinated with all other dental specialties in one NY dental office before the start of treatment so patients will get the result they expect.
It is usually healthier to treat crooked teeth with braces rather than with Cosmetic Dentistry (without braces) because Orthodontics actually corrects the problem.
The drawback to Orthodontics is the time involved, interim aesthetics and the inconvenience. At CSD each orthodontic treatment plan is compared with many Cosmetic Dentistry options and patients are offered assistance in making an appropriate choice.

Our staff Periodontist is the dental specialist who typically treats gum disease in concert with our dental hygienists. At CSD your gum therapy will be coordinated with all other needed dentistry at the same time. We carefully personalize your Periodontal treatment plan so that, for example, your Cosmetic Dentistry occurs in the appropriate sequence, after Endodontics and Periodontics.
The skills of individual dental hygienists vary as greatly as those of dentists.
High volume offices will typically schedule patients in 20 30 minute time slots whereas CSD will typically schedule hygiene patients for one hour. There is a reason our teeth cleaning visits are more expensive. If bacteria is left behind after a poorly performed cleaning then the ravages of tooth decay and gum disease may progress unnoticed by the naked eye for many years until the damage is done.
At CSD a combination of early gum disease diagnosis, early preventive therapy and bacterial cultures with species-specific antibiotic regimens should dramatically reduce the need for gum surgery.
CSD is relatively rare in providing this Oral Microbiology Testing but the reader will agree it is logical after a review of the literature on the National Institute of Health's U.S. Government website. The results we obtain using conservative methods are quite significant and therefore our need for severe gum surgery is dramatically reduced.

There are several ways to make your teeth whiter
Bonding, Porcelain Veneers (Laminates), Porcelain Crowns (Caps) and Teeth Whitening (Bleaching). At CSD the method of teeth whitening is carefully considered based upon each patient's specific dental condition and cosmetic requirements. Only natural teeth structure responds to whitening so people should seek an experienced Cosmetic Dentist for whitening. Whiter teeth may cause your old porcelain crowns, veneers and/or bonding to look fairly yellow in comparison. At CSD this knowledge and experience is considered in your personal treatment plan.
Office whitening is preferable to home whitening because of the fast results and patient convenience.
One main reason for the better results with the office procedure is because of the duration of time that the whitening agent is in direct contact with the teeth. Patients typically come in for one or several forty-five minute appointments depending upon the amount of color change desired. The whitening agent is applied directly to the teeth while a lip retractor holds the lips away from the teeth and a suction drains excess saliva. There are no shots or drilling. Patients will commonly relax with Nitrous Oxide gas and music during the procedure. Significant color change is frequently apparent after the first visit.
Many companies falsely promote the benefits of using laser light and/or their own special brand of light to increase teeth whitening.
Actual scientific research shows no added benefit. The only factors that increase teeth whitening are the chemistry of the whitening agent, the duration that the agent is in contact with the teeth, and the skill and experience of the Cosmetic Dentist. Obstructive sleep apnea (OSA) is a very common sleep disorder that affects tens of millions of Americans.
It is characterized by interrupted breathing during sleep, resulting in an insufficient delivery of oxygen to the lungs. At the individual level, OSA is associated with an increased risk of cardiovascular disease, high blood pressure, stroke, diabetes, and arrhythmias. MRI-based studies have even shown that patients with OSA had memory loss and decreased sizes of certain brain structures, such as the mammillary bodies.
But Sleep-Related Problems Go Beyond the Individual Patient.
Not only are undiagnosed and untreated sleep disorders dangerous to your own health, they are also a burden on our overall society. Excessive daytime sleepiness costs the economy $150 billion in terms of lost workplace productivity, while motor vehicle accidents involving drowsy driving costs another $48 billion annually.
The latest report by the Institute of Medicine (IOM) estimates that as many as 70 million Americans suffer from chronic sleep disorders
Including obstructive sleep apnea, daytime sleepiness, and snoring. That's 1 in 5 Americans! In addition, the national institutes of health (nih) states that 12 million Americans today have obstructive sleep apnea, although this is a conservative estimate. The NIH recognizes that many cases of sleep apnea are unreported because people either do not report the condition or do not know they have the disease.
Thus, it is important for all patients to be screened for sleep disorders to help prevent the abovementioned health complications.
Sleep disorders include sleep apnea, snoring, narcolepsy, insomnia, and chronic fatigue.
Dentists with special training in sleep medicine play a vital role.
After a diagnosis is made by a sleep physician, a dental sleep medicine provider can custom design and deliver oral appliance therapy (OAT), a proven treatment option for those with sleep apnea. For example, a mandibular advancement device (MAD) can be calibrated to a certain distance, thereby creating an ideal airway opening for a patient with snoring or OSA. Other devices can stabilize the jaw and tongue, as well as increasing the muscle tone of facial structures.
Typically, OAT is indicated for patients with mild to moderate sleep apnea, whereas those with severe OSA rely on heavier, bulkier face-mask devices called CPAP (continuous airway positive pressure).
Unfortunately, these large devices can severely affect the quality of life of users and their partners and family members. In addition, some patients undergo throat surgery (i.e., Uppp uvulopalatopharyngoplasty) to help alleviate symptoms. While there are various effective treatment modalities, many patients prefer that doctors offer conservative options such as oat before surgery or cumbersome face-mask machines are considered.
With oral appliance therapy, dentists can alleviate or eliminate the many symptoms associated with sleep apnea.

General and Children's Dentistry

There are many factors that affect the quality of even simpler cosmetic dentistry.
For example, there can be nearly a dozen different materials used in a single tooth-colored Bonded filling: tooth etchant, which microscopically roughens the tooth surface for bonding; primer, which further roughens the tooth surface, dentin/enamel bonding resin with a light or dual cured catalyst, possible use of self-cured posterior composite and bonding materials. The quality of each of these materials can vary tremendously. The best brands benefit the patient by minimizing post-operative pain, resisting the recurrence of tooth decay and breakage, and maintaining color-fastness over time. The top brand materials used at CSD are among the most expensive to buy and the most time-consuming to use. The results are worth it. CSD does not place silver amalgam fillings.
It is common to see patients who have had two, three or even four separate fillings, performed at different times, and placed within the same tooth!
'Patchwork' dentistry should not be done because decay is usually present under these older, adjacent fillings. At CSD we usually remove the old, adjacent silver fillings within that same tooth when placing new white bonded fillings. This takes more time, but being thorough now will save our patients from needing more dentistry sooner rather than later.
CSD only works with New York Lab Technicians who hand craft beautiful, well fitting Crowns (Caps) that don't show any dark metal on the biting or on the inside surfaces.
When necessary a CSD Porcelain Ceramist will be present for final porcelain color shading and characterization. There are many benefits to working with local NY Master Ceramists rather than working with inexpensive dental labs in China. The skill in preparation and impression-making of your teeth for caps is significant. Crowns should also be made to minimize long-term gum problems (overly bulky caps irritate gums) and maximize cement retention (so the cap won't fall out). Dental impression material brands also vary in quality (and cost) so patients can learn about their dentists just from the brands used. All these factors allow CSD to create exceptionally beautiful cosmetic dentistry that can last a long time.
