EAP-Initial Session/FIRST EAP Appointment (1 hr)

Please bring the authorization number and the number of sessions that was given to you by your Employee Assistance Program, along with their phone number. These sessions are free to you, provided that all authorizations can be confirmed by Life Tree and all authorizations are received by the EAP company. Each approved session will be billed to your EAP. You may be required to sign additional paperwork given by the EAP at the first or second session.

Initial Individual Evaluation Appointment- 90791

This is the first visit to is designed to discuss your specific situation. Together, we will assess whether we are an appropriate fit and determine appropriate treatment goals. LTCC will file insurance for you and will obtain proper authorization. We will review deductible and co-insurance fees that may apply on your initial visit. Your copays and coinsurance most likely have increased this year.
