Preparing for Surgery
There are a number of important steps to take when preparing for surgery. By closely following your plastic surgeon's instructions, you can set yourself up for a successful procedure and the quickest, safest, and most comfortable recovery possible. When preparing for your surgery:
Stop smoking.
Smoking slows the healing process and increases the risk of complications.
Practice good nutrition.
Eating healthy foods both before and after surgery can help you heal more quickly and feel better.
Stop taking aspirin and ibuprofen.
Medications containing aspirin and ibuprofen can thin the blood and cause excessive bleeding during and immediately after surgery. Acetaminophen is a safe alternative.
Stop taking herbal medications and supplements.
Because these products are not thoroughly tested, they can interact unexpectedly with anesthesia, which could cause life-threatening complications.
Fill your prescriptions.

Implant Materials
Early silicone gel breast implants were not as good as today's breast implants. They were eventually abandoned in favor of saline-filled implants. Today, advances in breast augmentation technology have made safe silicone-gel-filled implants available, giving our Stockton / Modesto breast implants patients a choice between saline and silicone breast implants. Modern silicone implants are safer than earlier designs because they do not leak or rupture easily. While both saline and silicone implants produce excellent results, many patients choose silicone implants because they consider them to have a more natural look and feel. Saline implants, on the other hand, give Dr. Silverton more flexibility during breast enlargement surgery, as they are inflated after being implanted. They are also safer in the unlikely event of an implant rupture, as saline is simply saltwater.
Size and Shape
Implant size is one of the most important considerations for our Stockton and Modesto breast augmentation patients when planning breast enlargement surgery. Smaller implants tend to look the most natural, while larger implants offer the most dramatic breast enhancement. When choosing the ideal size of your breast implants, it is important to consider how larger breasts might affect your lifestyle. Extremely large implants can get in the way of certain athletic activities, such as running, and some women who choose larger implants may experience other problems due to the increased implant weight. Shape is also important. Breast implants are available in both round and teardrop shapes. Today, most women prefer round, smooth implants, because of their better appearance and softness. Dr. Silverton uses breast implants in a variety of widths and projections to achieve each patient's desired appearance.
Incision Location and Implant
Breast implants can be placed through a choice of incisions. Dr. Silverton generally prefers the peri-areola incision. This incision can be concealed well in the border between the areola and the surrounding skin, resulting in a better scar. It also gives Dr. Silverton more ability to control the shape of the breast and minimizes the implant lowering that is more common with the incision under the breast. Silicone implants require a slightly larger incision than saline filled implants. Sometimes they must be placed through incisions located in the crease beneath the breast known as the infra-mammary fold. This location allows for longer incisions but may leave more visible scars in patients with smaller natural breasts. Because they can be filled after placement, saline implants can be implanted through smaller incisions in a wider variety of locations. Dr. Silverton prefers to avoid the incision in the armpit. Dr. Silverton generally prefers to place breast implants behind the chest wall muscle rather than in front. The procedure to place the implant behind the muscle generally produces a better visual result. The scar is better and the long term shape usually holds up better. This option is also less likely to interfere with mammograms in the future and minimizes the likelihood of complications such as rippling and capsular contracture.
The Breast Augmentation Procedure
Breast augmentation usually takes between one and two hours to complete and is typically performed under general anesthesia administered by our board-certified anesthesiologist. After anesthesia has taken hold, Dr. Silverton makes the appropriate incisions and places the breast implants. Once the implants are in place, he carefully closes the incisions with dissolving sutures and secures the treated area with a light dressing and a comfortable bra. Patients recover under the supervision of skilled RNs and there is usually minimal discomfort. Patients are able to return home, or to a local hotel if they live some distance away, after they have recovered in the recovery room.
Recovering from Breast Enlargement
Recovery time varies for patients at our surgical center in Stockton depending on how the breast augmentation procedure was performed. In most cases, our breast implants Stockton / Modesto patients have their implants placed behind the chest muscles. For the first 24 to 48 hours after surgery, most patients experience mild discomfort, which can be controlled with pain medication. During this time, it is important to relax and get plenty of rest. Within ten days weeks, bandages can usually be discontinued. Breast enlargement patients leave our surgical center wearing a comfortable bra. No compression bandages are required. Within a week most patients can buy a bra of their own selection at a store of their choosing. Dr. Silverton will make recommendations. The breast implants settle into place over the next several weeks and the breasts soften. It is best to avoid exercise and other strenuous activity for about six weeks. Surgical scars may take several months to fade, and they never disappear completely. However, they are typically subtle and most patients consider this a small price to pay for breast enhancement.

Breast Lift with Implants
Patients whose breasts have recently begun to droop may benefit from breast lift with implants. Ideal candidates for this procedure are typically those who have lost some breast volume, often as a result of breastfeeding. Because the skin cannot shrink as the breasts lose volume, the breasts begin to sag and may appear flat. Patients who have lost volume in their breasts may benefit tremendously from a breast lift with breast implants, which can restore breast volume and projection. During the procedure, Dr. Silverton typically places the implant through an incision around the edge of the areola.
Breast Lift
As the breasts continue to droop, a more thorough treatment may be required. Breast implants alone may improve the shape of the breasts, but a lift may be necessary to return them to a more youthful position. For breasts that have just begun to droop, Dr. Silverton is able to perform breast lift by removing skin from around the areola. The remaining skin is reattached, lifting the breast into place. This type of breast lift, also known as a Benelli lift, is minimally invasive and leaves little visible scarring. For Stockton / Modesto breast lift patients with more severe drooping, Dr. Silverton may need to use anchor-shaped incisions that run around the areola, down the underside of the breast, and along the inframammary fold. This incision allows for more dramatic breast uplift, but also results in somewhat more scarring and a longer recovery time. As with the Benelli lift, excess skin is removed along the incisions to lift and firm the breast.
Recovering from Breast Lift Surgery
Recovery time among patients who visit us in Stockton, near Modesto and Tracy, varies according to the type of breast lift and whether breast implants are used. Patients who undergo a Benelli lift can expect the shortest recovery time, while those who undergo a full anchor-style breast lift incorporating breast implants can expect the longest recovery time. During the first week of recovery, it is normal to experience some discomfort, swelling, and bruising. These side effects can be managed with medication and typically subside within a month. It is important to avoid sleeping on your stomach and lifting anything over your head during this time, as pressure on the breasts could cause complications. About Oversized Breasts
Uncomfortably large breasts may be caused by several different factors. Whatever the cause, breast reduction can help correct the condition. For some women, large breasts are simply hereditary. Others gain breast volume due to pregnancy and breastfeeding. A hormonal imbalance due to a medical condition or medication could also be the culprit. Women with overly large breasts often suffer from insecurity and discomfort. In addition, many of these women experience constant back and neck pain. It is common to suffer from indentations and bruising on the shoulders from bra straps. Many also have difficulty exercising because their breasts get in the way.
About Breast Reduction Surgery
Breast reduction surgery takes about four hours to perform, so Dr. Silverton's patients are placed under general anesthesia for the procedure. At Trinity Plaza Surgery Center state-of-the-art surgical facility, conveniently located for patients in Stockton, Modesto, and Tracy as well as the Bay Area, breast reduction is an outpatient procedure. Breast reduction is typically performed using an anchor-shaped incision that runs around the areola, along the underside of the breast, and down the inframammary fold beneath the breast. Excess skin and tissue are then removed through this incision. Dr. Silverton adjusts the position of the breast to account for its new size and then carefully closes the incision, leaving the breasts more proportionate to the rest of the body.
Recovering from Breast Reduction
Though breast reduction is an outpatient procedure, it is still a major surgery requiring weeks of recovery. It is normal to experience some discomfort during the first days of recovery, but Dr. Silverton prescribes medication to help control it. Before leaving our surgical center, breast reduction patients are fitted with a post-surgical support bra to be worn throughout the recovery process. Most patients will spend a few days resting after surgery. Swelling and bruising are to be expected during the first few days after surgery, but will subside with the help of surgical drains to remove excess fluid. These drains are typically removed three days after the breast reduction procedure. It is a good idea to partake in some light activity, such as walking, as soon as possible after surgery, as this will help speed the recovery process by promoting circulation. However, breast reduction patients should avoid bending over, straining, and heavy lifting for at least three weeks after surgery. Most patients are able to return to work within a month depending on their work responsibilities and physical condition.

As a person's weight fluctuates, the skin stretches to accommodate the extra fatty tissue. Unfortunately, when people successfully lose weight, the abdominal skin typically does not shrink to the body's new shape; instead, the skin hangs in loose folds. The abdominal muscles can also become loose, particularly in patients who have been pregnant. This problem can be incredibly frustrating for those who are committed to looking and feeling their best, and it typically cannot be resolved without surgery. The Abdominoplasty Procedure

The abdominoplasty procedure takes about four hours. To begin the tummy tuck, the patient is placed under general anesthesia. Dr. Silverton makes one incision running from one hipbone to the other along the lower abdomen, and another incision around the navel to separate it from the abdominal skin. Dr. Silverton then uses special sutures to tighten the abdominal muscles. In some cases, he may perform liposuction to remove some residual fat; however, abdominoplasty is not a weight loss procedure and produces better results in patients who have already lost most of their excess weight. Finally, Dr. Silverton will remove excess skin from the abdomen before carefully closing the incisions with sutures.
Recovering from Tummy Tuck Surgery
In the early stages of tummy tuck recovery, patients should be prepared to rest at home, engaging only in light walking to increase blood circulation to the incision site. Some discomfort is normal, but this can be controlled with pain medication. Patients recovering from abdominoplasty will have surgical drains in place for up to two weeks in order to control swelling by draining away excess fluid. It is always important for tummy tuck patients to wear a surgical support garment throughout the recovery process in order to minimize strain on the incision site and help the skin adhere more closely to underlying structures. Most of our Stockton / Modesto tummy tuck patients are able to return to work within a month of surgery and resume more intense exercise within six weeks.

New and improved liposuction abdominoplasty (tummy tuck)
The tummy tuck procedure has evolved and improved since the procedure was first tried in 1899.
The standard technique, which has been with us for about 30 years, consists of a horizontal incision in the lower abdomen.
The skin and fat is lifted up off of the muscle. The belly button is saved by cutting around it. The muscles are tightened and the skin and fat flap is brought down and trimmed off.
This is still a fairly standard technique in most plastic surgery practices. It has a couple of problems that make this method rather risky.
First, there may be insufficient blood getting to the lower central skin after the procedure and this can result in an area of dead skin which can create a hole.
This might take weeks or even months to heal up with intensive wound care. Liposuction at the same time as this technique makes this problem worse. As a result we avoided liposuction at the same time. This resulted in residual abdominal fat that would need liposuction as a second procedure.
The second is that there is a large space under this flap that needs prolonged drainage tubes and often drainage fluid will accumulate there after the drains have been removed.
This is called a seroma, and the fluid needs to be removed by new drains or needles.
I have been doing a better technique recently which seems to have solved these problems.
Research has shown us that the blood supply for the lower abdomen comes through deep blood vessels in the upper outer portion of the abdomen. If this area is preserved then healing problems become rare or non existent. Liposuction can be done at the same time and healing still remains very good. So now we can do complete liposuction of the abdomen and then go ahead and do a modified tummy tuck to preserve these blood vessels.
The results have been amazingly good.
The fat is gone. The stomach is tightened more than we could before. Healing is great. Drainage tubes come out in a week or less. Recovery is quicker. Pain is less. We use a pain pump to automatically numb the surgical area with local anesthetics for about four days. Skin numbness is less. Seromas have all but been eliminated. Return to normal activities is much quicker.
If you are extremely thin and you do not need the liposuction we can omit it and do the rest of the new and improved technique.

The Mini-Tummy Tuck Procedure
Mini-tummy tucks are typically performed under general anesthesia. To perform a mini-tummy tuck, Dr. Silverton performs extensive Modesto liposuction as required to re-shape the abdomen. Dr. Silverton then removes excess lower abdominal skin through a transverse lower abdominal incision, which is typically shorter than the incision used for a full abdominoplasty. Mini-Abdominoplasty Recovery
Because mini-abdominoplasty is significantly less invasive than a full tummy tuck, patients enjoy a much shorter recovery period with less discomfort. Patients are typically released from our surgical suite within a few hours of surgery so that they can go home to rest and recover. Any post-operative discomfort in the days following surgery can be controlled with pain medication. Most patients are able to return to work within two weeks. Activities, including heavy lifting and strenuous exercise, should be avoided for a few weeks. However, walking and other light exercise should be resumed as soon as possible after surgery to help speed the recovery process and minimize complications.
Mini-Tummy Tuck Benefits

How to decide what needs to be done to improve your tummy.
A tummy tuck is also known as an abdominoplasty. It is one of the most common cosmetic procedures performed in my practice.
Loose skin and stretch marks often develop after pregnancies.
Excess fat can accumulate in the tummy area with weight gain. When the fat is lost with dieting and exercise the extra skin remains. It might sag and form an area of overhanging skin. Stretch marks can look bad.
Often this is cosmetically unacceptable, but every patient is different. Your particular problem will be examined and evaluated individually. I will customize the procedure for you.
Although the anatomy of the abdominal wall is quite complicated it can be broken down to three distinct layers - muscle, fat and skin.
We need to decide if one or more of these layers is your particular concern.
If it is fat only, then liposuction might be appropriate. Liposuction will not tighten skin or muscles.
Liposuction
With liposuction I make small punctures - usually three. I inject some clear IV fluid containing epinephrine.
I treat the skin with ultrasound applied to the skin surface.
I suction using the syringe liposuction method where there is no machine. The vacuum of the syringe provides all the suction power that is needed.
When performed for the right Modesto / Stockton liposuction patient, this procedure can produce outstanding aesthetic results.
Mini Tummy Tuck (Mini Abdominoplasty)
If you have a moderate or small amount of fat and and also a limited amount of loose skin a mini abdominoplasty might be the right choice.
A mini abdominoplasty is liposuction plus skin removal from the lower abdomen. This will tighten the lower abdomen, below the belly button. The belly button is not moved with this technique and it acts as a tethering point making it impossible to tighten the upper abdominal skin with this method.
However, for some people whose extra skin is confined to below the belly button and whose upper abdominal skin does not need correction
and whose muscles are not too weakened or stretched, a mini tummy tuck or mini abdominoplasty might be a great choice.
With a mini abdominoplasty recovery is quick, and no drains are required. It also costs less.
However, most people do not fall into this category because the fat and extra skin and stretch marks are spread out throughout the whole abdomen
including the area above the belly button. If you are in this category, a full tummy tuck in Stockton might be the best answer.
Complete Tummy Tuck (Liposuction Abdominoplasty)
Recently the full tummy tuck and been significantly improved. The results are much better than they were before, and recovery is quicker.
All our tummy tucks are done as an outpatient. You go home the same day.
In order to explain the new technique and new improvements I would like to comment on the old technique first.
The old technique has been popular for the past 30 years or so. It consists of making a long cut across the lower abdomen where it will normally be covered by clothing. The skin and fat is lifted upwards off the muscles to the ribs. When the belly button is reached it is preserved by making a cut in the skin all around it. The skin of the stomach looks a bit like an apron as it is lifted upwards and there is a hole where the belly button used to be.
The muscles may have spread apart due to pregnancy.
This is corrected by pulling the muscles together again in the mid-line with stitches. This creates a flatter firmer abdominal wall and a slimmer waistline.
The skin and fat is now stretched downwards and the excess is trimmed off at the level of the original incision.
The incision is now closed after inserting drains. At this time the belly button is now covered by skin. A hole is made right over where the belly button is. It is located and brought through. Your belly button is now restored.
Problems
This is the old technique and it has three major problems:
The fat under the skin that remains has not been removed.
There is a large potential space where two large surfaces can rub together. This space needs to be drained and this space can cause problems.
There is a risk of skin dying just above the pubic area resulting in delayed healing or a hole that needs prolonged dressing care.
This is due to poor blood flow in this area. This risk is greatly increased in smokers.
Over the past several years we have done a lot to improve the technique and get around these problems.
We think that we have all of these problems solved now.
Now we do liposuction of the entire abdomen to remove the excess fat.
This has the effect of mobilizing the skin without cutting anything.
Then we lift the skin and the lower abdominal tissue to the level of the belly button, but not as high as before.
We save the belly button in the same way as before.
We create a tunnel upwards from the belly button to the upper abdomen so that we can tighten the muscles from the top to the bottom at the pubic area.
What we do not do is to elevate the skin to the ribs on both sides.
This preserves the blood flow to the skin above the pubic area. The skin can get tight and move down just as much as before because it is loosened by the effects of the liposuction. I can explain exactly how this works when I see you in person for the consultation.
So now the procedure is safer with much better healing, and better blood flow to the skin above the pubic area.
The actual surgery on your tummy is less. The drains stay in for a shorter time, because there is no longer a large potential space that needs to be drained. Recovery is quicker. Pain and discomfort is less. We use a pain pump to lessen the pain even more. …and best of all the results are very much better than before.

Or you might want to go the whole way and get the best possible tightening and go for the best result, and you accept that there are some more risks, and recovery can be longer than for the lesser procedure.

The most common side effects after using Latisse solution are an itching sensation in the eyes and/or eye redness. This was reported in approximately 4% of patients. Latisse solution may cause other less common side effects which typically occur on the skin close to where Latisse is applied, or in the eyes.
